• A copy (.jpg, .png, .pdf, .heic) of your Photo ID*
• Most recent copy of your DSHS award letter (example TANF award letter, HEN award letter). Please upload all pages of award letter.

*Approved ORCA LIFT agents, completing the form on behalf of a client, may verify photo ID in person instead of uploading a copy.
