Regional Reduced Fare Permit
For riders with a disability
How the provider-certification process works:
- Download and print the Reduced-Fare Permit Application form (PDF).
- Take the form to your health care provider and have them fill out the Health Care Provider section on page 2. We accept certification from any of the following Washington State Licensed providers:
- Physician (M.D.)
- Psychiatrist
- Psychologist (Ph.D.)
- Physician's Assistant (P.A.)
- Advanced Registered Nurse Practitioner (A.R.N.P)
- Audiologist certified by the American Speech-Language-Hearing Association
- Osteopathic Physician (D.O.)
- When your provider has completed the certification, gather your application documents:
- Health Care Provider Certification form
- A copy of your photo ID (driver's license, passport, or state-issued ID)
- A clear color photo of your face, to put on the permit. Follow the same guidelines you would for a passport photo, including:
- Photo must have been taken within the past 6 months.
- Photo size should be 2" x 2".
- Head must be between 1" and 1⅜" in size, from the bottom of the chin to the top of the head.
- Use a plain white or off-white background.
- Have someone else take the photo. (No selfies.)
- Take off your eyeglasses for the photo.
- No filters or other digital editing.
- Complete the online application form. You'll be prompted to upload your scanned documents when you've completed the form.
If you prefer, you can complete the paper form by hand and return your application materials by mail or bring them to our Customer Service Office or a Neighborhood Pop-up.
